Enter transaction SPRO in CRM and follow this path:
SAP Reference IMG -> Customer Relationship Management -> Transactions -> Basic Settings -> Settings for Activities -> Questionnaires.

You can then create new questionaires by clicking on "Define Questionnaires". When doing so make sure to:

1. Create the new questionnaires/surveys under the Activity folder.
2. Maintain the survey attributes as "Offline handheld" in the "Scenarios" tab.
3. Activate the Survey.

You will then need to Define Determination for Questionnaires (Activity). Here you will be able to select which activity transaction type will be associated with your survey, choose the validity period, as well as select whether the survey will be mandatory or optional.
